Appointment & Health Information: The reason for your appointment (e.g., main clinical concerns, reason for euthanasia), plans for the care of remains, previous primary care hospital, and any other medical information you choose to share.
Travel Information (for Health Certificates): Origin address, destination address, and planned method of travel.

To Create Health Certificates: To gather the necessary information to complete Interstate and International Health Certificates.
To Maintain Medical Records: To create and maintain accurate and complete medical records for your pet(s), as required by veterinary professional standards and California law.

Service Providers:
Squarespace (and Acuity): Our website host and scheduling platform. They process your data on our behalf when you book an appointment.
Google (Google Workspace): Our email and calendar provider. When you submit a form or an appointment is booked, that information is sent to our Gmail-managed email addresses and stored in our Google Calendar.
Third Parties for Veterinary Services (As-Needed):
Aftercare Services: If you request services for the care of your pet's remains, we will share necessary information with your chosen pet cremation or aftercare provider.
Regulatory Bodies: We may be required to share information with the California Veterinary Medical Board or other governing bodies as required by law.
B. Data Storage and Security
We use commercially reasonable administrative, technical, and physical safeguards to protect your personal information. Your data is stored on the secure platforms of our service providers (Squarespace and Google), which use industry-standard security measures.
However, please be aware that no method of transmission over the internet or electronic storage is 100% secure. While we strive to protect your data, we cannot guarantee its absolute security.

Our services are not intended for or directed at children under the age of 13. We do not knowingly collect personal information from children under 13. If we become aware that we have, we will take steps to delete that information.
